This diagram shows the basis why to donate post endexpiratory pressure (peep) to avoid an alveolar collapse (& consecutively atelectasis) during mechanical ventilation. LaplaceĀ“ law describes the relationship between pressure, radius, wall tension & layer thickness in a sphere. As is well known, surfactant reduces wall tension at the end of expiration, as well (not demonstrated). The best peep approximates the lowest possible pinsp and so optimally reduces driving pressure!
